New Patient Registration Procedure

Procedure

If you would like to register as a patient with us, we require you to complete the following registration forms. You can collect all the forms from our reception or download them from the links below. Once completed please bring them along to the practice with 2 forms of ID, one with an address and one with a photograph for example a utilities bill and a passport.

NHS Form – this form is required by the NHS to ensure that your medical records are moved from your previous practice.

Ethnic Origin Form – this form is voluntary but will help the NHS in Scotland provide a better service.

Medical Questionaire – this form is used collect contact details and medical information that will give us a complete record to help provide you with the best possible service.  Please provide as much information as you can especially regarding any medication as it can take some time for your records to reach your new practice.

Alcohol Screening Questionnaire – this form is an ongoing initiative to look at alcohol intake.

Vaccination Record – this is an additional form for patients under 5 years of age.

SMS consent form - we are finding that sms text messaging is a very effective way to communicate with our patients over 16 years of age, we require your consent  to do this.
